Sometimes while doing various moderation tasks (e.g. moderating the daily giveaway), a moderator will notice a reviewer is doing something dishonest (e.g. committing plagiarism in their giveaway entry comments).

When this happens, it will be helpful if the the moderator can use a default miscellaneous form to ban the reviewer. The reviewer would also need the option to send a message to the moderator to appeal the decision, such as in a case where a moderator makes an accidental human error.
