I am noticing that when client's report a problem with a review that earned a great score (e.g. 100%) from the editors, the responding staff member often does not bother to warn the editor who scored the review so well and approved it, despite obvious issues with it.

In fact, this failure itself is what leads to future reviews being posted with similar issues. If one review goes by without an editor being warned, then that same editor will continue making the same mistake, and what would have been one poorly written published review turns into 10 then 100 all because one staff member didn't take the time to issue a warning to the editor who is not doing the job correctly.
